Payments, co-pays, and deductibles due at time of service.  A $50 fee will be assessed for no call/no show appointments.  Please verify with your insurance carrier if you need authorization and that your deductible has been met before your visit. 

A sliding scale is available for the uninsured and under insured.

$60 to $80 for the under insured or uninsured (Must provide proof of income at time of service).
